Prominence Health Plan Reports Data Breach |
Prominence Health Plan says a data breach earlier this year may have exposed some people's information.
Officials say the breach was discovered in late April.
They add the data accessed did not include Social Security numbers or any financial information. Instead, it was audio recordings from the Prominence call center, files with healthcare claim forms and letters to patients with claim approval or denials.
Prominence is sending notices to any impacted members and offering free credit monitoring and ID theft services.
Prominence is providing notice to approximately 45,000 individuals representing all members of the Prominence Health Plan during the 2019-2020 timeframe.
Prominence is implementing additional security measures and reported the incident to the FBI as well as federal and state regulators.
Plan members will receive letter notifications with an offer for free credit monitoring and ID theft assistance services, as well as other recommendations that can be taken to protect against the potential for identity theft and fraud.

Individuals may call (888) 829-6550, Monday – Friday, 6:00 a.m. to 8:00 p.m. Pacific Time, or Saturday – Sunday, 8:00 a.m. to 5:00 p.m. Pacific Time (excluding U.S. national holidays).
